Appointment times: Appointments will start and end at their scheduled times. In consideration of other patients, sessions cannot be extended for late arrivals.
Cancellation policy: A full session fee will be charged for cancelled or missed appointments unless at least 2 business days of notice has been given. Both telephone and email are acceptable ways to alert Dr. Ozbolt of a cancellation.
Initial Adult Consultation: 60 minutes – $450
Initial Child/Adolescent Consultation: 90 minutes – $600
Follow-up Medication Management: 30 minutes – $400
All other professional services requiring longer than 10 minutes must be completed through regularly scheduled appointments. Crisis interventions (such as extended telephone sessions) as well as extended paperwork requirements or other consultations not completed in session will be charged a pro-rated fee based on Dr. Ozbolt’s rate of $600 per hour. No medical prescriptions will be provided over the phone. In the event of any emergency or crisis, call 911 immediately.
Payment: All outpatient visits must be paid in full at the time of visit. If payment is not received, your credit card will be charged at the time of the appointment. Dr. Ozbolt is not a member of any managed care insurance programs, but a standard one page invoice can be provided for patients to submit to their insurance carriers. Dr. Ozbolt accepts checks, cash, and all major credit cards for all professional services. There will be a fee of $50 for checks returned for insufficient funds.
Insurance Reimbursement: Dr. Ozbolt is not a part of any insurance panels and does not process requests for insurance reimbursement. As such, she is considered an “out of network” provider for most PPO plans. If you have a health benefits policy that provides mental health coverage, you may be entitled to insurance reimbursement for any provided professional services. Potential reimbursement is between you and your insurance carrier, and you may discuss this with your insurance company by contacting them directly. Regardless of the patient’s ability to receive insurance reimbursement, full payment for all services is required at the time of each appointment, as outlined above.
Phone and e-mail contact: Dr. Ozbolt will return routine phone calls and e-mails free of charge. However, if returning phone calls and emails for a particular patient becomes frequent and time-consuming, Dr. Ozbolt may ask the patient to increase their frequency of appointments in order to address these issues during session times. Time incurred for lengthy phone calls (e.g., longer than 10 minutes) to gather collateral data, update other individuals (such as family members or other care providers) about the patient’s treatment or for other purposes may be billed to the patient at Dr. Ozbolt’s discretion and charged to the patient’s credit card held on file. In the event of an emergency, please contact 911 immediately and proceed to the nearest hospital emergency room; you may leave a follow up message for Dr. Ozbolt at 305-202-2100, but due to the nature of outpatient practice, it may not be possible to respond immediately. Therefore, if a situation requires an immediate response, please call 911 or go to the nearest hospital emergency room. Please be aware that e-mail is not a confidential means of communication.
